Permissions & Emergency Contacts
Please list any other person(s) as emergency contacts and those authorized to pick up your children.
1.) I understand that my child may participate in physical activities such as those held during games and recreation. As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal responsibility, Green Level Baptist Church, their representatives, staff, and volunteers.
2.) In the event of an emergency that requires medical attention for the above-listed child(ren), I understand every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my permission to the Green Level Baptist Church staff and/or volunteers to secure the services of a licensed medical professional or emergency medical personnel to provide the care necessary for my child's well being. I assume responsibility for all costs associated with any accident or treatment of my child(ren).
3.) I grant permission for a photo of my child to appear in an unpublished directory to be used by Green Level staff and volunteers. I give my permission for photos of my child(ren to appear among general club photos in print or online as long as no identifying information is included.

Parents not serving as mentors will have a Bible Study related to parenting.
Wednesday Night Meal Plan
This year we are offering a pay-in-advance option to contribute to Wednesday night meals one time per semester. To contribute, please fill out the
Level Up Meal Plan form.
This donation helps offset the cost of food and supplies associated with Wednesday night meals.
Pay week-by-week options will be available later.
Please select the number of children you are registering for the correct payment amount. If you would like to sponsor a child or add an additional donation, choose to pay for one more child than you are registering.
If you prefer to pay with cash or check, choose that option from the payment drop-down menu and see the Level Up Team to pay. Thank you!
