Please list any other person(s) as emergency contacts and those authorized to pick up your children.

1.) I understand that my child may participate in physical activities such as those held during games and recreation. As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal responsibility, Green Level Baptist Church, their representatives, staff, and volunteers. 

2.) In the event of an emergency that requires medical attention for the above-listed child(ren), I understand every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my permission to the Green Level Baptist Church staff and/or volunteers to secure the services of a licensed medical professional or emergency medical personnel to provide the care necessary for my child's well being. I assume responsibility for all costs associated with any accident or treatment of my child(ren).

3.) I grant permission for a photo of my child to appear in an unpublished directory to be used by Green Level staff and volunteers. I give my permission for photos of my child(ren to appear among general club photos in print or online as long as no identifying information is included.
